We're new to the area. How can I prepare for our first appointment with your office?

New patient packets are available on our website that you may download, print, fill out, and bring with you to your first appointment. If you have any records from your child’s previous healthcare provider(s), please bring copies—or at least their contact information so we may request it from their office directly. If you’re unable to fill out the packet ahead of time, please arrive early to your appointment to fill them out prior to your visit.
